Health Plans, Office Policies & Financial Information

I accept most major health plans, insurances and Employment Assistance Programs. Otherwise, there is a set fee for each session. Please contact me for information regarding your particular situation.

You can leave messages 24 hours a day at (219) 629-3669. I require 24 hours notification of a canceled appointment. Otherwise you will be billed for the session since most insurance plans and Employment Assistance Plans do not cover missed sessions. Again, if you have any question, please feel free to contact me.
